MINIBUS DRIVER APPLICATION FORM
Please be aware that there are a number of conditions attached to our insurance which mean that we need the correct details to be given here. Only drivers who have completed this form will be allowed to drive the minibus. Once having completed the form, any changes to the information below should be notified to us as soon as possible (i.e. points on licence). Please include the name of the Group using the minibus, below, if different from the driver’s name.
Conditions of use:-
Drivers must have been driving for at least 5 years, or must hold a minibus driving qualification. This qualification must have been held for a year and evidence must be shown before the bus can be used.
We will need to have a photocopy of your driving licence and one other form of ID (eg. utility bill) so that your name age and address can be checked.
Drivers must be at least 25 years of age.
The booking can only be confirmed 3 months before the date of use. This allows time for church bookings to be notified, which will take preference.
